OUR DUTY OF CARE FOR ALL OUR CLIENTS WITH OUR QUALIFIED AND EXPERIENCED CASE MANAGERS ON-CALL IS TO LISTEN, AND TO ASSIST WITH YOUR CONCERNS
ALL CLIENTS THAT ARE REGISTERED WITH RECIPROC8 PATHWAYS WILL BE OFFERED FREE COUNSELLING CASE MANAGEMENT SERVICES
REGISTERING AS A RECIPROC8 PATHWAYS CLIENT
THE PROCESS:
1) BOOK AN APPOINTMENT WITH OUR CASE MANAGEMENT TEAM.
BEFORE OUR CASE MANAGEMENT TEAM CAN OFFER ASSISTANCE, AN INTERVIEW MUST BE CONDUCTED TO IDENTIFY WHAT SERVICES WILL MEET THE CLIENTS NEEDS. ELIGIBILITY OF GOVERNMENT FUNDED COURSES WILL BE ESTABLISHED IN THE FIRST INTERVIEW.
2). FOLLOW UP INTERVIEW
A REVIEW WILL BE CONDUCTED AND A CONFIRMATION OF THE CLIENTS GOAL PLAN WILL BE NEGOTIATED BETWEEN THE CASE MANAGER AND THE CLIENT.
3). ACTION THE GOAL PLAN
CASE MANAGERS WILL BE WORKING CLOSELY WITH INTERNAL STAKEHOLDERS TO ENSURE THE CLIENT IS PROGRESSING PRODUCTIVELY.
4). MONTHLY FEEDBACK AND PROGRESS REPORTS
OUR CASE MANAGERS WILL BE REVIEWING AND WILL RELAY FEEDBACK TO CLIENTS REGARDING PROGRESS.
